Camp Cadiz Student Registration

PLEASE READ BEFORE COMPLETING THE REGISTRATION FORM! Students must have completed the 6th grade or higher to participate. Students currently in the 12th grade, graduating in May of the current year, should still complete the student registration form. Students are preferred to sign up with a church that has an adult leader participating.

Students and a parent/guardian should complete the registration form together. You will need your insurance & medication information to complete. The form will ask for demographic and medical information and questions to help place the student in a work area for the week.

During camp check-in, a printed copy of the registration form will be provided. A PARENT/GUARDIAN MUST COME IN TO REGISTRATION AND SIGN THE FORM.

You can use NONE for any question(s) that does not apply to the student.

How the name is entered on the form, is how it will be on the student shirt. For example – if you put Robert on the form, but you go by Bob, Robert will be on your shirt. It is recommended that you put Bob (Robert) in the name. It will also be spelled how you put it in the form so if you, put Maggggie, that is how it will be on your shirt. Please double check before submitting.

Make sure the correct size is indicated ADULT or Youth (small child).

Service area – Select something different for each of the 6 options – parents/guardians – PLEASE have your student assist with this section. Every year we have parents who completed the form and the kid is not where they would like to be.

We try to honor your first or second choice, but it is determined on the projects submitted and adult volunteers that we have.
